We need to upgrade the existing 2013 SP farm which is running 2008 R2, we have many servers with their own roles. Also, our farm is customised with WSP, APP model, Hybrid Search, ACS and with many third Party tools. We can see that we have the option to do slipstream and upgrade the farm, need to get all the details which might occur during and post upgrade.

The Microsoft Knowledge Base article 2818510 describes the standard procedure to execute
such an upgrade.
Anyway, I would recommend not reusing the same server as per that article: SharePoint is difficult to completely uninstall, I think the best solution would be adding new servers running Windows Server 2012 R2 to the farm on a one by one basis and then remove
the old ones running Windows Server 2008 R2 after each new server is active in the farm.
