2. I set up a simple, out-of-the-box Task List w/ no custom workflow from our Team Site.
3. The Team Site is using default permissions of: Team Members | Everyone except external users
4. "Require content approval for submitted items?" is set to Yes
5. "Send e-mail when ownership is assigned?" is set to Yes
6. Other e-mail notifications (such as sharing a site) seem to work
7. If I assign a task to me, I receive the email on some lists, but not others.
8. If Ia assign a task to someone else, they don't receive the notification, but used to.

If it is a permissions issue, it would be very helpful for someone to indicate exactly what minimum permissions are required for the site and/or list. Instead of just saying "Sounds like a permissions issue to me." Not very helpful to a new user.
