使用客户端验证的自定义列表表单

我在自定义列表新表单中有以下详细信息

i have below details in the custom list new form

用户说明:(选择字段）

user description:(choice field)

用户全名:(单行）

用户地址:(单行文字）

user address:(single line text)

此外，我需要按照上述格式添加10个用户列表详细信息（最多）我有相应的user1描述，user1全名，user1地址.... ,,,, user10描述等。所以它应该是用户友好的，这样用户可以相应地添加/删除。

further i need to add 10 user list details (at max) as per above format and i have corresponding user1 description,user1 full name,user1 address....,,,, user10 description so on respectively.also it should be user friendly such that user can add/delete accordingly.
